    These forms are located in each classroom, grades 3-5.

     

     

    "I NEED HELP" FORM

     

    Students (grades 3-5) can request help from the counselor anonymously by submitting an "I Need Help" form to her mailbox.  

    Grades 3 and 5--please submit your "I Need Help" form to Ms. Wong's mailbox

    Grade 4--please submit your "I Need Help" form to Ms. Hooper's mailbox

     

    Younger students (grades PreK-2) can discuss problems with their teacher and then the teacher will solicit our assistance if needed. 

    Any student has access to a school counselor per teacher referral, or a request from the student. 

    Similar to the school nurse, students have access to the school counselors when and if needed.

    However, if the child requests regular counseling meetings, we will send home a permission slip for the parent to sign for individual counseling.
