  • Drug Testing Information:
    Drug testing is required for all students in order to become a TSV Employee. The address for this location is:

    飩 Appointments are not required but they are preferred. If you choose not to make an appointment, walk-ins are welcome only until 3:00 pm. To make an appointment please call the number listed below.
    飩 The two page document with the Center for Occupational Medicine logo must be filled
    out and brought with you.
    飩 Please bring a form of identification with you; student ID, license, etc.
    飩 If you are under the age of 18, you must have your parent or legal guardian sign the paperwork.
    o Please note that if you are under the age of 18 and your results are positive, your parent will be contacted.
    飩 If you are under the age 18 and do not have the paperwork filled out ahead of time, a parent or legal guardian must be present to sign upon arrival.
    飩 Failure to complete and pass a drug test during this week will result in disqualification from the RPM program.
