All paperwork will now be uploaded into the DragonFly app. Davidson athletics will be fully paperless. Please reach out to your coach if you need help using the app or refer to the apps FAQ's and tutorials. You will print this packet, fill out completely, upload your documents into DragonFly and then wait for Coach K to approve all of the paperwork.

PLEASE TURN IN A PHYSICAL ON THE 2019 VERSION. IF THE DOCTOR HAS THE 2010 VERSION PLEASE GIVE THEM THE UPDATED FORM!!!!

Entire packet MUST be filled out prior to any athletic activities. Insurance information, driver form mustbe included. This is the only physical form allowed. Any other version of this form will not be accepted.
